Port Chester and Blind Brook schools are closed for winter recess thru Feb. 23. Please pick up an intake form in advance at the Information Desk or arrive 1/2 hour early on your appointment day (if your appointment is after 9 a.m.). Please bring the following: a copy of last year’s federal and state income tax returns; W-2 forms showing income from your employer; any 1098 or 1099 forms (examples: 1099-R, SSA-1099); any other information concerning your income and deductions; photo ID and Social Security/ITIN cards; NYS Driver License or ID Card (NYS residents) and bank information if direct deposit of refund or withdrawal of taxes owed is desired.
